There are numerous insurance plans available, therefore, it’s impossible for our staff to know the covered benefits of each plan. We will do our very best to help you understand your coverage based on your individual healthcare plan.
Ultimately, it is your responsibility to know and understand the policies and benefits of your plan, including referrals, authorizations, copayments, deductibles, covered hospitals, labs, and x-ray facilities.
In order for us to bill your insurance, we ask that you provide a copy of your current insurance card at the time of the service. If you do not provide the necessary insurance information for billing, you will be required to pay for your visit at the time of service.
Notice to Patients:
– Effective May 1st, 2014 we are no longer accepting CA Medi-Cal health insurance.
– Effective July 1st, 2014 we are no longer accepting American Express for payment.
– We accept Cash, Checks, Visa or MasterCard only.
